Activity Director Resources and CME Activity Application

If you would like to apply for and plan a traditional CME activity or Internet CME resource, please complete the CME Activity Application and Planning Form and email as an attached document to: professionaleducation@hss.edu along with all supporting documentation. Your application will be reviewed within 30 days of receipt and scheduled for approval by the hospital's Continuing Medical Education Committee. Before completing the form, we encourage you to review the Activity Director's Resource Guide on how to plan, design, implement and evaluate a CME activity.
